File a Complaint

To file a complaint about smoking at a worksite, yours or someone else's, please complete the following form and either fax it or mail it to us. The Tobacco Control Program will research all complaints, forward the complaint to the relevant enforcement agency, and track complaints to identify repeat offenders.
